The cause of this sort of problem can be difficult to pin down, you just have to eliminate them one by one.

Anything suggested so far is a possible cause as is a worn mandrel and bushes with too much slop in the fit.

You can check if the mandrel is bent by removing it from the collet, placing it on a flat surface such as a machine bed and see if it rolls true. If it is bent it should be obvious.
