Parts working loose

Some time ago, I made a Streamline pen for my wife, that she keeps in her handbag. A few months ago she complained that the clip was turning round, so I pressed the finial back in and thought no more about it. Now it has happened again and there is also a gap between the nib and the body. It seems the pressed in parts are working loose somehow. I could maybe understand the nib, but she never uses the clip at all! Is this something you guys have seen before, and is the solution to glue these parts in?

During the pressing indications of firmness can be felt or corrected. I use a rod to hold the brass for abrading if this is too tight I pitch the brass tube right there, too slack same treatment I am thinking another spare tube measured would solve the too loose problem or glue in carefully at the time.

Peter.
